In considering the application, we have consulted with the Centers for Medicare & Medicaid Services (CMS) and have reviewed and approved the proposed sections of the MFCU's agreement with the State Medicaid agency. This approval is valid for 3 years from the date of this letter. Under the regulations, the MFCU may request renewal of this approval for additional 3-year periods by submitting a written request to OIG. Please ensure that you include the outcomes of your data mining activities as part ofthe MFCU's annual report that is due by November 1, 2014, as outlined in section 1007.17 (i) of the regulations. .
