The #VA #OIG evaluated a May 2023 power outage at the Hines Information Technology Center, which lasted approximately 22 hours and prevented thousands of VA employees from accessing critical VA data and systems. We found that the center’s infrastructure design includes a circuit breaker that functions as a master power switch between the uninterruptible power supplies and the information technology equipment. Consequently, when the circuit breaker was activated it interrupted the flow of electricity going to the data center equipment and critical applications hosted at the facility. The Department of Veterans Affairs, Office of Inspector General has revealed a fundamental way to improve care. It has to do with how medical staff assess patients about to be discharged. The OIG has recommended a national policy to use social determinants of health, and health-related social needs, when evaluating patients. The Federal Drive with Tom Temin spoke to the U.S. Department of Veterans Affairs deputy assistant IG julie kroviak about why that's important.

The #VA #OIG determined the actions taken by El Paso VA Health Care System leaders, including those initiated before the VA OIG inspection and those implemented after VA OIG inquiries, ensured that the system’s behavioral health clinic staff did not deny patients access to care, and that patients were seen in the time frame and at the location that met their preferences and needs.
