Issues With ADLs Increasing Consulting Group ATI Advisory reports that over 15% of community-based Medicare beneficiaries are struggling with activities of daily living (ADLs) such as bathing, toileting and walking. It says these signals the need for long-term supports and services, especially in marginalized communities. As we know, chronic long term care is not a feature of Medicare per se. Medicare long term care is limited to short-term stays post hospitalization. Medicare Advantage, however, now help in this area with numerous new supplemental benefits on the market. This came about about good and creative regulations from CMS that expanded supplemental benefit offering areas. But the provision of chronic long term care largely is the province of Medicaid and unfortunately we do not have strong national policies on long term care. Some progressive states excel, but in many states access to alternatives to nursing home institutionalization are not strong. The middle class is especially in a bind. Its only choice often is to spend down assets and get on Medicaid when family members can no long support them at home. Lawmakers and policymakers need to get their arms around this issue and create a national aging policy. Not done so will only mean our costs grow for both expensive nursing home care in Medicaid as well as acute care costs because health goes unmanaged when long term supports are not there. A bill that would expand eligibility for MinnesotaCare, allowing anyone to buy into the program as part of a public option on MNsure starting January 1, 2027, was heard in the Senate Health and Human Services Committee on March 14. Supporters of the public option argue that it is needed to address the severe problem of underinsurance caused by patients purchasing high-deductible products that do not provide adequate coverage. Opponents argue that adding more Minnesotans to public programs that pay low reimbursement rates will increase financial pressures on clinics and hospitals.
