A critical question is what proportion of patients, revenue, or margin stems from fee-for-service (FFS) versus VBC contracts. In light of the infrastructure opportunities highlighted by Peter Pronovost MD, PhD, FCCM, it's evident that health systems are still striving to achieve a cohesive integration of their care models, business operations, and compensation frameworks. Health systems engaged in significant risk contracts face difficulties in modifying their operating models, addressing the variability in hospitalization rates, and improving affordability outcomes. Conversely, systems with fewer risk contracts at times subsidize low-volume hospital-at-home programs, where financial incentives are not aligned with desired outcomes. Additionally, large health systems encounter formidable challenges in aligning executive compensation with the diverse factors that contribute to organizational value across different models. In my humble opinion and from my experience, I believe the true difficulty is landing interviews, being given a chance to learn and excel in these positions, and being paid equitably...not necessarily finding job openings. This has definitely been my experience and that's one of the reasons why I started my own business. I figured if organizations refused to see my value, I would take matters into my own hands, network, and provide services I know I am qualified to provide for the most vulnerable populations around me! I decided 6+ years ago not to allow someone's "No" to deter me from being great or to diminish my worth! Many public health professionals like myself have advanced degrees, certifications, and years of experience; yet, we are constantly told what we have accomplished is not good enough. Imagine how that constant rejection can make someone feel. I'll tell you how it makes them feel...like their accomplishments and hard work mean absolutely nothing and were for nought. I stopped playing the victim a long time ago and when I did, I met some amazing people, one of which is the Extraordinary LaTonya Steward-Bynum, BS, MPH, CHES®, MCHES®!! We were trying to figure this thing out and the only thing that made sense was promoting ourselves through our own businesses and marketing our resources as consultants to organizations who value our worth! I urge organizations everywhere to take a chance on public health professionals looking for work! We are intelligent, more than capable, and the very nature of our field requires that we be able to apply the concepts that we learn (through our practicum experience). Further, while you're giving us a chance, please pay us what we deserve and do not lowball us. I know public health professionals whose pay qualifies them for SNAP benefits and Medicaid Expanded! #InboxMe 😐 Thank you again for posting this.
