There isn’t just one type of Medicaid but several. Each type of Medicaid has its own specific eligibility guidelines. Knowing what type of Medicaid you need and how to apply for it can be a confusing process. Luckily, there are several different methods to apply for Medicaid:
- WV PATH: Website where you can apply for DoHS assistance programs online including SNAP and Medicaid. WV PATH is the recommended method of application for most types of Healthcare.
- Health Insurance Marketplace: Federal website to apply for West Virginia Medicaid, WVCHIP and standard health insurance with tax credits.
- 24-hour federal call center helpline – 1-800-318-2596 or TTY 1-855-4325
- Local County Offices: You can visit your local county office to apply for WV DoHS assistance programs including Medicaid.
- If you have any questions about the Medicaid application process, contact your local DoHS office for guidance.
- Customer Service Center Hotline: Call the DoHS hotline at 1-877-716-1212 for questions, guidance, to report a change in your situation or to apply for some types of Medicaid and WVCHIP.
- Applications - Print and mail completed application to your local county office.
- All-Programs Application –This application can be used to apply for most types o f DoHS assistance programs including all types of Medicaid and SNAP.
- Use this application if you are applying for multiple benefits (SNAP and Medicaid) or you aren’t sure exactly what type of Medicaid you need.
- When using the All-Programs Application, you will also need to complete the Rights and Responsibilities.

- Healthcare Application for Families – Application for Medicaid and WVCHIP for multiple individuals living together. (Spanish Version)
- Healthcare Application fo r Individual – Application for Medicaid and WVCHIP for only one person.
- Consider the "short" form because only one person can use it to apply.
- If you complete either applications and are denied Medicaid or WVCHIP, the eligibility worker will see if you are eligible for any other type of Medicaid. In this situation, you may be asked to complete another form, collects additional asset information.

- Medicare Premium Assistance Application – Application to specifically apply for Medicare Premium Assistance Programs.
- There are three different types of Medicare Premium Assistance Programs:
- Qualified Medicare Beneficiary (QMB) – Covers Medicare Part A and Part B premiums and Medicare co-pays and deductibles.
- Specified Low-Income Medicare Beneficiary (SLMB) – Covers just the Medicare Part B premium.
- Qualified Individual (QI-1) – Covers just the Medicare Part B premium.
- While a person can apply for Medicare Premium Assistance using the above methods, the Medicare Premium Assistance application is the preferred method to apply for QMB, SLMB or QI-1.
If You Receive Supplemental Security Income (SSI)
If you are receiving a check from Supplemental Security Income Program, you are automatically eligible for Medicaid and should receive a medical card from the West Virginia Department of Health and Human Resources.
If You DO NOT Receive SSI
If you are not receiving an SSI check, you may apply for Medicaid benefits using any of the options above. Applications are taken weekdays at your local office of the West Virginia Department of Health and Human Resources.
Most local hospitals and primary care clinics have staff available to assist you in making application.
If, because of a physical handicap or disability, you are unable to go to the local office, you may request a staff person to visit your home and take the application. To request a home visit, call your local office of the West Virginia Department of Health and Human Resources or call the Office of Client Services toll free at (800) 642-8589.
